And Puerto Rico have passed comprehensive smoke-free indoor air laws that include e-cigarettes, as of March 31, 2023. These laws prohibit smoking and the use of e-cigarettes in indoor areas of private worksites, restaurants, and bars. In our fight to end youth and young adult nicotine addiction, we focus on the issues that matter most.
A Yale study in 2019 found that, among students at three Connecticut public schools, those who used e-cigarettes were more likely to smoke regular cigarettes in the future. Most e-cigarettes have a battery, a heating element, and place to hold a liquid (such as a cartridge or pod). The e-liquid usually contains nicotine, flavorings, and other chemicals. As the user draws on the device, the battery heats the e-liquid to produce aerosol (not water vapor).
The 2019 outbreak of EVALI cases emerged against a backdrop of an ongoing vaping epidemic among youth. The amount, quantity and toxicity of substances that e-cigarettes release depend on several factors, such as the type of device, how the device operates and is used, and the e-liquid used. Although e-cigarettes do not give off smoke like tobacco cigarettes, they do expose people to secondhand aerosol or "vapor" that may contain harmful substances. Scientists are still learning about the health effects of being exposed to secondhand e-cigarette aerosol. The FDA monitors the national usage rates for all tobacco products, including an annual youth survey, and has seen a drastic increase in youth use of e-cigarette products in recent years.
In regard to COVID-19 pandemic, the actual literature suggests that nicotine vaping may display adverse outcomes. Therefore, follow up studies are necessary to clarify the impact of e-cigarette consumption on human health in SARS-CoV-2 infection. Recently, a commonly commercialized crème brûlée-flavoured aerosol was found to contain high concentrations of benzoic acid (86.9 μg/puff), a well-established respiratory irritant [88]. When human lung epithelial cells (BEAS-2B and H292) were exposed to this aerosol for 1 h, a marked cytotoxicity was observed in BEAS-2B but not in H292 cells, 24 h later.
Beginning July 1, 2019, Illinois law prohibits the sale of tobacco products, including these products, to people younger than age 21. The smoke also caused damage to the lungs and bladders of mice, indicating that it may increase the risks of developing lung and bladder cancer. Since e-cigarettes arrived in the U.S. in 2007, they have been investigated by addiction researchers as possible cessation devices for adults trying to quit combustible, or regular, cigarettes.
The particles you inhale while vaping can cause inflammation (swelling) and irritation in your lungs. This can lead to lung damage like scarring and narrowing of the tubes that bring air in and out of your lungs. Researchers don’t yet know all the effects vaping can have on your body. Susceptibility to smoking or non-combustible nicotine use is also influenced by national tobacco control policies. In Europe, Finland has been among the countries with most advanced tobacco control regulations [31]. Still, there is a large difference in smoking and snus use between students in academically and non-academically oriented education [7]. For snus, Finland follows the EU-wide sales ban but cross-border traveller imports from Sweden are permitted and an illicit market exists [32]. For e-cigarettes, comprehensive regulations were introduced in 2016 including retail sale licensing and ban for distance sales, display and advertising at point-of-sale, other flavours than tobacco and use of e-cigarettes in smoke-free places [33].
According to the CDC, 1 in 4 deaths in the U.S. result from cardiovascular disease caused by cigarette smoking. Teens who smoke can end up with lungs that are smaller and weaker than the lungs of teens who don’t smoke. Damage to the respiratory system can also make you more susceptible to certain infections that affect the lungs, like tuberculosis and pneumonia, and increase the possibility of death from those illnesses.
When studying tobacco cigarettes, researchers rely on smoking machines that simulate how frequently a typical smoker takes a puff and how much smoke is inhaled with each breath. No one has yet determined how much e-cig vapor the typical user breathes in, so different studies assume different amounts of vapor as their standard, making it difficult to compare their results. Tracing what happens to that vapor once it is inhaled is equally problematic.

"This is the first study to demonstrate the effectiveness of any quit vaping program for adolescents. It’s also a program that has been used by more than 780,000 young people nationwide since Truth Initiative launched it in 2019," Dr. Amanda Graham, chief health officer at Truth Initiative and principal investigator of the study, said in an email. Her team tracked rates of both vaping and smoking among nearly 5,000 Columbus-area people diagnosed with lung cancer, and compared them to a group of almost 27,300 people without cancer. It found that there was not enough evidence to confirm its products did not harm public health. The FDA has banned the sale of flavored nicotine products in the US unless a company has received their expressed approval.
They then compiled these rates into one score to determine the 'healthiest' and 'unhealthiest' states for kids. Overall, recent figures from JAMA suggest more than 2million kids under 18 vape regularly, along with 17million adults. Combined with smoking, it may accelerate these effects, leading to osteoporosis. Nicotine causes blood vessels to tighten, which restricts the flow of blood. Smoking also raises blood pressure, weakens blood vessel walls, and increases your risk of blood clots. It can also damage the DNA in sperm, making it difficult to conceive and increasing the risk of miscarriage and certain birth defects.
Vaping can cause eye, throat, and nose irritation, as well as irritation in the respiratory tract. The nicotine in e-cigarettes can cause dizziness and nausea, especially in new users. E-cigarettes contain many of the same toxins as regular cigarettes, but they may have smaller amounts. Some brands also have much less nicotine than regular cigarettes or no nicotine at all. In our study, there were between-school differences, which appeared larger for girls than boys. In the univariate analyses, most school-level factors either increased or lowered susceptibility. Federal, state, and/or local governments need to set clear standards on environmentally responsible e-cigarette waste disposal and hold the industry accountable for adhering to them. E-cigarette manufacturers introduced a new tobacco product that soared in popularity with little thought on how to responsibly dispose of the resulting tons of e-cigarette waste. The group then performed a statistical analysis to determine group differences in vascular function before and after vaping.
Median odds ratio (MOR), which shows the difference between two respondents with identical student-level characteristics from two randomly chosen different schools, was calculated to express between-school variance on the odds ratio scale [36]. The analysis was not pre-registered, and the results should be considered exploratory. This was a large increase from 2019, when only 11.3% of people had ever used e‑cigarettes, and 2.5% were currently using them (Figure 1). The proportion of people who used e‑cigarettes daily in 2022–2023 was 3.5%, rising from 1.1% in 2019. In smokers who smoke tobacco cigarettes, blood flow increased modestly after cigarette inhalation and then decreased with subsequent stress. However, with those who vaped, blood flow decreased after inhalation at rest and also after handgrip stress. Finally, lung health effects are not expected to be seen for 20 to 30 years.

Due to what has been called an ‘epidemic’ of youth use of these products, FDA has prioritized prevention efforts. The agency has taken a multitude of actions to keep ENDS out of the hands of youth, from policy making to enforcement to education. While e-cigarettes can generally be a lower-risk alternative for adults who smoke cigarettes, the use of e-cigarettes is not risk-free. These products deliver harmful chemicals and contain nicotine, which is highly addictive. Moreover, given the harmful chemicals found in e-cigarettes, further high-quality research on both short- and long-term health outcomes is needed. The longer ENDS and other e-cigarettes are on the market, the more information we know about their impacts on health.
While vaping might help you quit smoking, it probably won’t help you quit nicotine altogether. The CDC has identified vitamin E acetate as a chemical of concern among people with EVALI. Vitamin E acetate is a thickening agent often used in THC vaping products, and it was found in all lung fluid samples of EVALI patients examined by the CDC.
