This data brief demonstrates that teens whose environments put them at risk for use of e-cigarettes are more likely to try them. The study, published on May 23, 2022, in Tobacco Control, is the first to look at the health care costs of e-cigarette use among adults 18 and older. The second most supported measure, strengthening restrictions on the advertising and promotion of e‑cigarettes, was supported by 82% of people. In 2019, only 67% of people in Australia supported strengthening these restrictions. The proportion of people neither smoking regular cigarettes nor using e‑cigarettes has remained stable (Figure 3).
Because of this and other studies, vitamin E acetate is considered the main cause of EVALI. Other chemicals found in nicotine- and THC-containing vaping products may also play a role in the condition. It is important to know that most e-cigarettes contain addictive nicotine. Smoking is the leading cause of lung cancer, and people who smoke are 20 times more likely than nonsmokers to be diagnosed with lung cancer. Tobacco products contain unsafe substances, from acetone and tar to nicotine and carbon monoxide. The inhaled substances can affect your lungs and the other organs in your body.
So, if you are vaping to quit smoking, you should aim to eventually stop vaping too. In 2022, UK experts reviewed the international evidence and found that "in the short and medium term, vaping poses a small fraction of the risks of smoking". Vapes are electronic devices that let you inhale nicotine in an aerosol, or vapour, instead of smoke. This is done by heating a solution (e-liquid) that typically contains propylene glycol, vegetable glycerine, flavourings and nicotine. Vapes come in many shapes and sizes but they have the same basic components, including a battery, sensor, and atomizer/ flavor cartridge.
It can’t hurt to talk with a doctor or other healthcare professional about the risks of vaping, especially if you already have a chronic health condition, such as asthma. More research needs to be done in order to understand the side effects of nicotine-free vaping. Some of these cellular changes have been linked to the development of cancer over the long term, though there’s currently no evidence to suggest that vaping causes cancer. The 2018 NAP report found substantial evidence that vaping causes cell dysfunction, oxidative stress, and damage to DNA.
Restrictions on importation, packaging and flavourings will come into effect in early 2024 (TGA 2023). Between 2012 and 2022, most Australian jurisdictions have amended their laws to further restrict the advertising and sale of e‑cigarettes (Department of Health and Aged Care 2023b). Recent reports have linked serious lung injury – and even deaths – to vaping. According to the CDC, cigarette smoking causes nearly 1 out of every five deaths in the United States. It’s possible that we don’t yet know all the physical effects of vaping. Numerous studies have been performed to evaluate the safety/toxicity of e-cigarette use both in vivo and in in vitro cell culture.
E-cigarettes first entered the market in 2007 and have evolved many times since then, with older versions designed to resemble regular cigarettes, cigars, pipes, and pens. E-cigarettes next evolved into "pod mods" with e-liquid sold in disposable pods. In 2015, the emergence of the brand JUUL took pod mods to the next level with high tech design, high-nicotine delivery, and youth- appealing marketing that hooked a new generation to nicotine. Without regulations, it is the "Wild West" for e-cigarette companies, says Stanton Glantz, director of the Center for Tobacco Control Research and Education at the University of California, San Francisco, and a self-described e-cigarette pessimist. He argues that given the paucity of health data, current indoor smoking bans should apply to e-cigs as well.
According to the 2023 National Youth Tobacco Survey, more than 2 million teens reported vaping nicotine, and there was an increase in current overall tobacco product use among middle school students (4.5% to 6.6%) and multiple tobacco product use (1.5% to 2.5%). They are not technically categorized as smokeless tobacco, so the Food and Drug Administration (FDA) does not regulate them as strictly as smoked tobacco products or combustible tobacco. Without long-term data, we cannot be certain if or how reduced exposure will prevent harm to one’s health. In Canada, most of the provinces require smokers to be 19 years of age to purchase cigarettes (except for Quebec and the prairie provinces, where the age is 18). However, the minimum age only concerns the purchase of tobacco, not use. Alberta, however, does have a law which prohibits the possession or use of tobacco products by all persons under 18, punishable by a $100 fine.
Nicotine salts can be used in refill liquids and in cartridges for closed systems. In March 2019, the Secretariat of the WHO FCTC issued an Information Note which compiled all Conference of the Parties (COP) decisions related to e-cigarettes. However, it has been reported that the heating process can lead to the generation of new decomposition compounds that may be hazardous [4, 5]. The levels of nicotine, which is the key addictive component of tobacco, can also vary between the commercially available e-liquids, and even nicotine-free options are available. For this particular reason, e-cigarettes are often viewed as a smoking cessation tool, given that those with nicotine can prevent smoking craving, yet this idea has not been fully demonstrated [2, 6, 7].

However, the FDA then paused its decision in July while it carries out an additional review on the company's products. Many physical retail locations neglect to check identification for customers, allowing children to access the devices. Like cigarettes, an American must be at least 21 years old to purchase a vape. They can come as vape pens — which are shaped like a pen or small tube with a tank to store e-liquid and batteries — or pod systems that are rechargeable and often shaped like USB sticks.
Kasza's group also pointed to dual use of cigarettes and e-cigarettes, which has increased over time and usually involves smoking cigarettes more frequently than using e-cigarettes. "Individuals who dual use remain exposed to cigarette smoking toxicants, making complete rather than partial substitution of cigarettes with e-cigarettes important for risk reduction," they wrote. "Indeed, healthcare clinicians are now encouraged to discuss e-cigarettes as a cessation tool with patients who smoke cigarettes and have already tried FDA-approved cessation medications." A comparison of the serum levels of nicotine from e-cigarette or conventional cigarette consumption has been recently reported [39].
From early 2024, stronger controls on the importation, manufacture, advertising, supply and commercial possession of all e‑cigarette devices and components (with and without nicotine) are being implemented by the Australian Government. These regulatory changes are being implemented by the Federal Government in stages during 2024 in partnership with State and Territory Governments (TGA 2023). Researchers said that the results show e-cigarettes may confer as much as or potentially even more harm to users and especially patients at risk for vascular disease. Results showed that total cholesterol and LDL or "bad" cholesterol was higher in sole e-cigarette users compared to nonsmokers.
Enrollees in the program receive interactive daily text messages tailored to their sign-up date or their target quit date if they set one. Program users who set a quit date (which they can change) receive messages for at least one week prior to the quit date and for at least two months following the quit date. Those brands have been sold in the U.S. for years, awaiting FDA action on their scientific applications. To stay on the market, companies must show that their e-cigarettes provide an overall health benefit for smokers, without significantly appealing to kids.

In addition, nicotine use can affect areas of a young person’s brain that are responsible for attention and learning. Primary spontaneous pneumothorax, or collapsed lung, occurs when there’s a hole in the lung through which oxygen escapes.
Despite that, e-cigarettes are more popular among U.S. teens than any other form of tobacco. Electronic cigarettes are battery powered devices that people use to heat liquid into a vapor that can be inhaled. Some studies have shown that vaping by some youth may be linked to later use of regular cigarettes and other tobacco products. Using e-cigarettes may play a part in some kids or teens wanting to use other, more harmful tobacco products. Although the term "vapor" may sound harmless, the aerosol that comes out of an e-cigarette is not water vapor and can be harmful. The aerosol from an e-cigarette can contain nicotine and other substances that are addictive and can cause lung disease, heart disease, and cancer.
On the other hand, there is a large body of evidence clearly showing that FDA-approved medications are safe and effective ways to help people quit smoking, especially when combined with counseling. Scientists are still learning about how e-cigarettes affect health when they are used for long periods of time. It’s important to know that the aerosol ("vapor") from an e-cigarette contains some cancer-causing chemicals, although in significantly lower amounts than in cigarette smoke. The FDA has the authority to regulate all tobacco products, including e-cigarettes. The FDA is working on several options to prevent youth access to e-cigarettes.
However, the long-term effects of all flavour chemicals used by this industry (which are more than 15,000) remain unknown and they are not usually included in the product label [78]. Furthermore, there is no safety guarantee since they may harbour potential toxic or irritating properties [5]. In addition to its toxicological effects on foetus development, nicotine can disrupt brain development in adolescents and young adults [44,45,46]. Several studies have also suggested that nicotine is potentially carcinogenic (reviewed in [41]), but more work is needed to prove its carcinogenicity independently of the combustion products of tobacco [47].
To vape is to inhale vapor created from a liquid heated up inside a device. The devices have many names—vape pens, pod mods, tanks, electronic nicotine delivery devices (ENDS), e-hookahs and e-cigarettes. Most e-cigarettes contain nicotine and their repeated use can lead to nicotine addiction or increase the user's risk of addiction. As of 2018, all tobacco products must bear the required nicotine addictiveness warning statement on product packages and advertisements in order to warn consumers about the presence of and addictive nature of nicotine.
The last wave analyzed took place during the COVID-19 pandemic in March to November 2021 and was administered via audio computer-assisted self-interviews and telephone interviews. The study featured 1,985 participants; 49.4% were male and the average age was 40. The patient population was 80.7% white, 11.4% Black, 9.2% Hispanic, and 8% of another race. Truth Initiative forcefully rejects, however, the notion that this requires the further development of a huge commercial market in addictive nicotine products focused on growth and the acquisition of new users, most of whom are youth and young adults. Because the youth e-cigarette crisis in the United States and the youth appeal of flavored e-cigarettes go hand in hand, Truth Initiative strongly supports removing all flavored e-cigarettes from the market, regardless of device type. Based on the evidence already collected, the question shouldn’t be whether e-cigarettes are less dangerous than cigarettes, Christiani says, but whether vaping can be very harmful to health too. "To protect public health, I discourage the use of vaping—even to quit smoking," he stresses.
The most important points to know are that the long-term health effects of e-cigarettes are still unknown, and all tobacco products, including e-cigarettes, can pose health risks to the user. The tobacco industry has co-opted the term harm reduction to further its own interests of reduced tobacco product regulation and expand its user base. E-cigarette use among adults has been more stable, with adult use remaining relatively low since around 2012. Recent data, however, showed a slight uptick in adult e-cigarette use from 4.5% in 2021 to 6% in 2022. While switching completely from cigarettes to e-cigarettes can be less harmful than smoking, many adult smokers use e-cigarettes and cigarettes concurrently — increasing overall exposure to nicotine. Randomized control trials provide growing evidence that e-cigarettes with nicotine can increase quitting rates when compared to nicotine replacement therapies.
Nicotine-free e-juice contains a number of potentially toxic chemicals, such as base liquids and flavoring agents. However, based on the experience with cigarettes, similar adverse health effects, including COPD, heart disease, and cancer, can be expected. Finally, a 2017 review of the cardiovascular effects of vaping indicates that e-cigarettes may pose certain risks to the heart and circulatory system, notably for people who already have some form of heart disease.
A growing body of research indicates that truth campaigns to prevent young people from vaping are poised to move in the same direction as the organization’s successful smoking prevention campaigns. Let's say you love the feel and flavor you get from puffing on a vape — but you can't quite bring yourself to trust that it's safe. It uses a puff of pressurized, flavored air to simulate the "throat hit" of a vape, with no heating element whatsoever.
The evidence for e-cigarettes as a tool to help adults quit smoking is limited. To date, no e-cigarettes have been approved by the FDA as smoking-cessation devices. The report also discusses the changing landscape of tobacco products, including smoked products, smokeless products, and e-cigarettes. The liquid solution used in e-cigarettes, sometimes called vape juice or e-liquid, usually contains nicotine and other chemicals.
While their manufacturers say they are an alternative for tobacco smokers who want to avoid inhaling smoke, critics say too little is known about the safety of ESDs because they are not regulated. Beginning August 1, 2019, Minnesota law prohibits the use of these products indoors where cigarette use is prohibited, including bars and restaurants. A 2021 study found that daily e-cigarette usage among tobacco smokers can increase the likelihood of quitting smoking eightfold. Researchers assessed data from the 2014–2019 Population Assessment of Tobacco and Health (PATH) study, focusing on smokers who were not planning to quit smoking at the start of the period. However, a 2019 study into the long-term health effects of vaping found that people using e-cigarettes had a higher risk of respiratory disease than people who never smoked. The authors first created a comprehensive database of tobacco product flavor prohibition and restriction laws across the United States, including both state and local statutes. To date, there is no state excise or special tax placed on e-cigarettes.

She explains that even before CDC guidelines on treating the illness were released, Yale doctors were successfully treating patients with a combination of antibiotics and steroids. Doctors at Yale Medicine also collaborate with researchers in the fields of tobacco and addiction medicine to provide care for patients with EVALI.
Much like free-base nicotine products, these liquids contain propylene glycol and/or vegetable glycerin, as well as other chemicals such as flavouring ingredients. Most vaping devices use electrical power from a battery to heat a liquid solution. The term "vaping" is also used to refer to the inhalation of cannabis aerosols, which are produced by products that differ from nicotine products described on this page. Propylene glycol and glycerin are the most common solvents used in vapor products. While generally recognized as safe for ingestion by the FDA, research has found both substances to be toxic when aerosolized through the vaping process.
Parental smoking, positive attitude towards e-cigarette use in one’s age group, current smoking, and snus use increased S-EC, whereas liking school lowered it. At school level, both among girls and boys, a higher proportion of students liking school and being never-users lowered S-EC, whereas a higher proportion of students with positive attitudes towards e-cigarette use increased it. Among girls, a higher proportion of girls in the school, students with a high parental education and planning for GUSS lowered S-EC, but a higher proportion of students with parental smoking increased it.

New national laws to strengthen controls on the importation, manufacture, and supply of all e-cigarette products are now in place. This is an alternative way of cutting down your nicotine use until you are ready to stop completely. Remember to keep vapes and e-liquid out of the reach of children and pets, as there is a risk of poisoning if nicotine is swallowed. It's important to choose an e-liquid with enough nicotine to reduce withdrawal symptoms and urges to smoke.
Participants took one vape from an e-cigarette, with at least 12 mg/mL of nicotine, or inhaled a conventional cigarette, every 20 s for 10 min. Blood samples were collected 1, 2, 4, 6, 8, 10, 12 and 15 min after the first puff, and nicotine serum levels were measured by liquid chromatography-mass spectrometry (LC–MS). The results revealed higher serum levels of nicotine in the conventional CS group than in the e-cigarette group (25.9 ± 16.7 ng/mL vs. 11.5 ± 9.8 ng/mL). However, e-cigarettes containing 20 mg/mL of nicotine are more equivalent to normal cigarettes, based on the delivery of approximately 1 mg of nicotine every 5 min [40].

The campaign works to counteract the "cost-free" mentality held by many adolescents regarding e-cigarette use, showing them how e-cigarettes, just like cigarettes, puts them at risk for addiction and other health consequences. The campaign, ‘Every vape is a hit to your health’ utilises powerful creative to encourage young people to consider the proven health harms of vaping, such as nicotine addiction, lung damage, breathlessness, nicotine poisoning and burns from exploding vapes. If you're pregnant and need support to quit smoking, licensed nicotine replacement therapy products like patches and gum are the recommended option. Making small changes to your vape products or how you vape should help. Side effects are usually easily managed and should not stop you from vaping as a way to quit smoking. You're roughly twice as likely to quit smoking if you use a nicotine vape compared with other nicotine replacement products, like patches or gum.
Please be advised that many of these stories contain graphic and upsetting photos of injuries. Exposure to nicotine can also rewire a young brain to become more easily addicted to other substances, including cocaine and alcohol. Even if a cartridge doesn’t contain nicotine, other harmful chemicals may be present.

A cigarette is a narrow cylinder containing a combustible material, typically tobacco, that is rolled into thin paper for smoking. The cigarette is ignited at one end, causing it to smolder; the resulting smoke is orally inhaled via the opposite end. The term cigarette, as commonly used, refers to a tobacco cigarette, but the word is sometimes used to refer to other substances, such as a cannabis cigarette or a herbal cigarette. A cigarette is distinguished from a cigar by its usually smaller size, use of processed leaf, different smoking method, and paper wrapping, which is typically white. Not everyone is aware that there is nicotine in most e-cigarettes in varying amounts. Some pods of e-cigarettes contain as much nicotine as one pack of cigarettes (Truth Initiative, 2019).

E-cigarettes are not safe for youth, young adults, and pregnant women, and can be dangerous for adults who use tobacco products. The novel results for school-level factors indicate that prevention of nicotine use may be supported by facilitating nicotine-free schools through policies and programmes. This may address especially the peer selection processes, which have been observed both in school disengagement and smoking initiation [28, 29]. Further, it may influence the students’ descriptive and injunctive norms, which both predict smoking initiation among youth [27].
Their actions should come as no surprise as e-cigarette manufacturers fail to provide consumers with guidance or take responsibility for appropriate disposal methods. In a separate study conducted by Truth Initiative in 2019, almost half (46.9%) of e-cigarette device owners said that the e-cigarette device they used currently did not provide any disposal information, such as where to send used batteries or empty pods. Additionally, when e-cigarette device owners were asked about e-cigarette waste disposal, the majority (73.7%) believed that it was difficult to find e-cigarette drop off sites. Principal investigator Wendy Max, PhD, director of the Institute for Health & Aging, noted that from 2013 to 2018, e-cigarette use among high school students soared from 4.5% to 20.8%. The Australian Government’s new Public Health (Tobacco and Other Products) legislation commences from April 2024 and incorporates new controls governing the advertising and promotion of e‑cigarettes (Department of Health and Aged Care 2024). According to the National Tobacco Strategy 2023–2030 (Department of Health and Aged Care 2023b), strengthening regulations on e‑cigarettes has been listed as a priority area, and actions such as prohibiting the sale of flavoured e‑cigarettes have been proposed.
